Agnes' father was King Boleslaw ("Wrymouth", "Crooked Mouth") III of Poland and her mother was Princess Sbislava Svyatopolkovna of Kiev.  Her paternal grandparents were Prince Wladyslaw I Herman and Princess Judita I of Bohemia; her maternal grandfather was Prince Svyatapolk Mikhail II of Kiev and her maternal grandmother is concubine. She had a brother named "the Exile". She had two half-brothers and three half-sisters, named "the Great", Kazimierez, Richiza, Judyta and Agnieszka.  She died in 1181.

‌Agnes and Mstislav were married in a religious ceremony in 1151.  They had a son named Mstislav.
