Hazel Jane was born on September 22nd, 1897 in IN. Hazel Jane's father was John A. Sweezy and her mother was Martha Bernice Toothman. Her paternal grandparents were Daniel Sweezey and Mary Jane Gary; her maternal grandparents were Joseph TOOTHMAN and Sarah EVANS. She had four brothers and four sisters, named John A, Perry Edgar W., David Hershel, William, Leora, Daisy E, Myrta and Rena Bell. She was the second youngest of the nine children. She died at the age of 30 on February 4th, 1928 in Greensburg,Decatur,IN. She was buried on February 6th, 1928 in Southpark cemetery.-
