Underwood and her maternal grandmother is Julia A. Hazlewood. She had four brothers and three sisters, named Icye Lee, Billie Joe, Lindsey B., Kenneth Clarence, Lou Etta, Zelma and Dortha. She died at the age of 72 on February 26th, 1999 in LaRue Co.,Kentucky. She was buried in Otter Creek Baptist Church Cemetery,LaRue Co.,Kentucky.-
