Lewis was born in 1868 in Raleigh County, West Virginia.1 Lewis' father is James Wilson Cooke and his mother was Sarah Meadows. His maternal grandparents were Ephraim Meadows and Eleanor Dick. He has four brothers named Elijah, Jasper Francis, Josiah and Newton Isaac. He is the youngest of the five children. He has a half-sister named Sarah.-
