Foulques V of Jerusalam was born in 1092 in , Anjou, France.  Foulques V of Jerusalam's father was Ct. Foulques IV of Anjou and his mother was Bertrada Montfort.  His paternal grandparents were Ct. Geoffroi II of Gatinais (Ferreol) and Ermengardeof Anjou; his maternal grandparents were Ld. Simon Montfort and Agnes of Evreux. He had a brother named Ivo. He had a half-brother and a half-sister, named Foulques V of Jerusalam and Ermengardeof Anjou.  He died at the age of 51 on November 10th, 1143 in Plains of Acre, Israel.  He was buried on November 13th, 1143 in Jerusalem, Isrea.
