David Lee Phillips 
David was born in 1947. David's father was Harry Wayne Phillips and his mother was Elizabeth Helen Rodgers. His paternal grandparents were James Smith Phillips and Catherine Lavina Humphrey. He had three brothers, and also a sibling of unknown gender named Robert, Jack, Harry and <Unknown>. He died at the age of 40 in 1987.1
