Hazel was born on December 15th, 1918.1   Hazel's father was Martin D. Johnson and her mother was Tena Pladson.  Her maternal grandparents were Osten Halvorsen Pladson and Hage Midboe. She had a brother and a sister, named Gilman Johannes and Maxine.  She died at 9 months old on September 25th, 1919.1
