Jan Hendrik was born on September 30th, 1828 in Borculo.1  Jan Hendrik's father was Evert Kamphuizen and his mother was Janna Holmer.  His paternal grandparents were Jan Kamphuizen and Geesken ter Meul; his maternal grandparents were Hendrik Holmer and Maria Klein Dengerink.  He was an only child.  He died at the age of 68 on May 26th, 1897 in Borculo.2
