Reb Zusha's father is R' Yisroel Horowiz and his mother is Channa.  His paternal grandparents are HaChoize M'Lublin and Tehilla Shprintzah; his maternal grandparents are  Reb Dovid and  <Unknown>.  He is the third of four children.  He has two brothers and a sister, named R' Shmuel, Reb Dovid  and Tzviyah.
