Samuel's father was Philip Millichap and his mother was Harriett Matilda Andrews. His paternal grandparents were Henry Milichap and Eliza Chamberlain; his maternal grandparents were James Andrews and Ann. He had a brother and two sisters, named Philip, Rose and Alice. He was the second oldest of the four children. He died at the age of 53 in 1942 in Crosby, Lancashire.¹
