Forest Willyard 
Forest was born on September 14th, 1891 in Kentucky.1 Forest's father is Ambrose Willyard and his mother is Sara D. Patterson. His maternal grandparents are Hayden Patterson and Margaret Unknown. He was an only child. He died at the age of 84 on December 21st, 1975 in Owensboro, Daviess Co., Kentucky. He was buried on December 23rd, 1975 in Elmwood Cem., Owensboro, Kentucky.1 1 General Notes
