Forrest Avery was born in Dezarn, Green Co., Kentucky.1 Forrest Avery's father was John Avery Warren and his mother was Mary Fanny McGlasson. His paternal grandparents were Felix Grundy Warren and Elizabeth Frances Dearen. He had four brothers and three sisters, named Lewallen, William E., William Joseph, Alberta, Martha E., Mattie May and Alberta Lydia. He had three half-brothers and three half-sisters, named Ballinger Wright, John Taylor, Robert Lee, Ada Holt, Lula May and Stacy Jane. He died on December 11th, 1979 in Greensburg, Green Co., Kentucky.1 General Notes
