- NSW Birth Cert No. 26259/1895 states that Samuel HAYWOOD was born in the Redfern area of Sydney. His father's name was Thomas HAYWOOD and his mother's name was Ada HAYWOOD (nee Stacey).

Samuel's father was Thomas Haywood and his mother was Ada Stacey. His maternal grandparents were John Stacey and Mary Ann Brophy. He had a sister named Hilda. He was the younger of the two children. He died at the age of 72 on April 25th, 1967 in Laurieton, NSW.²

Death Notes
- NSW Death Cert No. 22930/1967 states that, in 1967, Samuel Frederick HAYWOOD died in the Taree area of NSW. His father's name was Thomas HAYWOOD and his mother's name was Ada HAYWOOD (nee Stacey).
  
  The Ryerson Index has an entry that states...
  "Samuel Frederick HAYWOOD died on the 25th April, 1967. aged 72. the Death notice also states that he died at Laurieton, and that he was formerly of Kogarah, Sydney. This notice appeared in the Sydney Morning Herald Death Notices on the 28-Apr-1967."
