Jan Hendrik was born on September 15th, 1899 in Barkly East.  Jan Hendrik's father was Jan Hendrik x van Blerk and his mother was Johanna Isabella Sofia Horne (van Blerk). He had a brother named Hendrik Jacobus.  He was the older of the two children.  He died at the age of 47 on February 28th, 1947 in Addington Hospital, Durban.  He was buried in Durban, Stellawood, Cemetery and Crematorium.
